  • Clinical Information: SPOP collects information that you share voluntarily by completing a clinic-related form, including the Initial Inquiry Form or the Professional Referral Form.  This information is subject to HIPAA and governed by the SPOP Notice of Privacy Practices (available upon request)If you share information about another person, you are representing to us that you have the authority to do so.
  • Donor Information: SPOP collects donor information that you share voluntarily through the fields at the “Make a Gift” section of our Website, including the “Join Our Supporters” page. When you use our online donation portal you are connected directly to secure third-party vendors to process this information. We never store any credit card data on our servers.
  • Bill Payment Information: SPOP uses a third party to collect bill payment information via the “Pay Your Bill” link. When you click this link, you are connected directly to secure third-party vendors to process this information. We never store any credit card data on our servers.
  • General Website Visitor Information: SPOP also collects some information as you navigate the Website. Automatic data collection includes browser cookies, which enable you to access our full website, and web beacons, which enable SPOP to measure traffic to different pages of our site. The information collected automatically does not include personal information.
